Our specialist respiratory Nurse delivers our pathway across the PCN; GP’s refer into this pathway for Patients with a new diagnosis of COPD/Asthma or complex Patients.

Hypertension Service
The Network Hypertension Project aims to find and follow up abnormal blood pressure readings as per current NICE guidance.
Cancer Project (DES)
As a Primary Care Network, we have signed up to an NHS England Directed Enhanced Service (DES) for Cancer, which involves supporting early diagnosis of Cancer. This contract requires us to develop processes for patients to improve the rate at which we can diagnose cancer.
Our PCN will have a Non-Clinical and Clinical lead appointment to this project, who will be working together to improve the Cancer Pathway to our Patients.
COVID Phase 3 Vaccination Programme
Following confirmation of the covid-19 vaccination booster programme by the joint committee of vaccinations and
immunisations (JCVI) we are pleased to be supporting the delivery for patients registered with one of our four primary care
network (PCN) practices.
