Bournemouth East Collaborative Primary Care Network (BEC PCN) has a new and exciting opportunity for an ANP/ACP to join the BEC Enhanced Care Team to work 15-30hrs per week providing acute visits as part of our multi-disciplinary visiting team. Wednesday, Thursday and Friday are key required days for the team, Monday and Tuesday would be preferable but not essential.

This role will require an Enhanced DBS Clearance. Should you have this certificate as part of the DBS subscription service, we would be happy to accept this providing the original is shown. Otherwise, you will be expected to complete a new application.

Job Summary

Our Enhanced Care Team is a multi-disciplinary team based at Southbourne Surgery that provides support to our aligned Care Homes, our proactive frailty support and acute visiting service to housebound patients.

We would ideally like an ANP/ACP to work alongside the Lead GP and Lead ANP for the Enhanced Care Team, deputising for the Lead ANP when required. The role will predominantly involve delivering acute visits to our housebound patients but will also involve offering training and support to our aligned Care Home staff, as well as a broader education and development for the communities served by the PCN.

Advanced Practice is a level of practice rather than a type of practice and the ANP/ACP within Bournemouth East Collaborative PCN will have the freedom and authority to act, making autonomous decisions in the assessment, diagnosis, and treatment of patients.

ANPs working at this level of education and training have significant roles within each of the following four pillars, and will be expected to help develop the role within the organisation: –

  • Advanced clinical practice
  • Leadership
  • Facilitation of education and learning
  • Evidence research and development

Main duties of the job

  • Able to use an advanced level of professional accountability, autonomy and judgement for seeing treating and assessing patients who present with a broad range of situations and conditions.
  • *You will have an advanced practice qualification and be a non-medical prescriber.*
  • Plan and manage complete episodes of care, working in partnership with other members of the multi-disciplinary PCN team, delegating and referring as appropriate, to optimise health outcomes for patients.
  • Work autonomously, clinically examining and assessing patients and initiating any appropriate action whilst taking responsibility for managing practice.
  • Using an advanced level of professional accountability, autonomy and judgment for highly complex decision making across a broad range of situations.
  • Have a level of understanding of PCN and NHS values, policies and procedures.
  • Provide support to Care Home Staff, which may include delivering training where required, or supporting Care Home pilots, and support the other members of the Enhanced Care Team.

Patient / Client Care

  • Takes responsibility for providing care and treatment in line with current evidence base practice at an advanced clinical practice level.
  • Demonstrates the ability to make critical judgements and utilise problem-solving skills, regarding patient care, on a daily basis.
  • Initiates and carries out highly complex patient assessment, analyses and interprets results, using highly developed advanced knowledge and skills that includes Comprehensive patient history taking, physical examination and understanding self-competences when interpreting range of tests and or investigations.
  • Independently prescribes a range of medications, as agreed at PCN level, based on self-competencies, efficacy, safety and cost.
  • Discusses and agrees assessment outcomes with patients, carers and other health professionals, to enable them to make informed decisions regarding treatment and care, and initiates appropriate, timely consultation and/or referral, when more expert input is required.
  • To comply fully with Infection Prevention and Control Policies

What we need from you

Essential

  • Professional registration with relevant professional body.
  • Recognised Advanced Clinical Practice course (or working towards) – MSc or Evidenced capabilities through portfolio route.
  • Advanced Practice Digital badge. Digital badge is for those who have an MSC have evidenced their capabilities through portfolio route
  • Non-medical prescriber.
  • Ability to see and treat a wide range of minor injury and illness presentations.
  • To take a full comprehensive history, clinically physically examine and treat within level of own competence.
  • Willing to learn clinical new skills to enhance practice.
  • Evidence of on-going CPD.
  • Have use of own vehicle with business insurance and clean full UK driving licence.

Desirable

  • Interpretation of X-rays.
  • Understanding of the Primary Care Network.
  • Additional skills of assessing elderly care.
  • Teaching and developing staff.
  • Ability to analyse and interpret complex information/results from patient assessment

  • Excellent interpersonal and communication skills, using a range of methods, e.g. verbal, written, record keeping.
  • Maintain sensitivity at all times to the emotional needs of the patient and their relatives.
  • To be competent to deal calmly and professionally with patients, relatives and work colleagues in a confidential and sensitive manner.
  • Able to work autonomously whilst using clinical decision-making skills at an advanced level.
  • Demonstrable ability to work in a busy environment and cope when the service is under pressure.
  • Takes pride in their work and have a strong sense of personal and team accountability.
  • Strong sense of commitment to openness, honesty and integrity in undertaking the role.
  • Good organisational skills.
  • To be flexible and multi skilled to ensure the priorities of the PCN can be met.

Desirable

  • Computer literacy with a range of software, databases, spread sheets.
  • Has an understanding of SystmOne.
  • Is able to complete task, two week wait referrals and templates.
  • Seeks out, and takes, opportunities for improving the service offered, with customer focus in foresight.

Experience

Essential

  • Has previous experience working at an advanced level in primary care, urgent care or emergency care.
  • Evidence of working at an Advanced level.
  • Experience of managing a range of primary/urgent care conditions for adult and paediatric patients. Demonstrates a willingness to do telephone triage.
  • Reflect on own practice and direct own learning

Desirable

  • Previously worked as part of an integrated team
